Pro-Abundant Life (PAL) Pack Nomination Form
Forms must be submitted by one of the Foundation's church partners
We would love the opportunity to bless a few of our Northwest Ohio neighbors with a package of gifts we pray will add joy to their life and allow our generous supporters to be pro-ABUNDANT life. If you know someone in your congregation or community that is a senior citizen, we would like to consider blessing them with a PAL pack that would include some items we hope will give them things to do, watch, read, learn from and/or connect with others. The goal of a PAL pack is to give them something other than basic care items. Rather, a PAL pack would have items that add to an abundant life. Also, each PAL pack would include a Bible, either a children's Bible or a large print Bible, so that they also are blessed with the opportunity to have life into eternity and a walk if abundance in this life. As a partner with the Foundation for Life, we would love to work through you to put these PAL packs into the hands of people that could use more joy in their life.
Each nominee can include a couple of preferences and special requests when agreeing to be a PAL recipient. They can indicate a list of a few things that are a nice to have as an extra item but that would not include large items such as furniture or appliances, clothing, home decor and the like. Preferences can include color, flavor, topics of interest or something along the lines of helping us to narrow the options of books, puzzles, movies, snacks, hygiene products and similar items. Examples of special requests: a lap blanket, a new bed pillow, a scarf or hat, slippers, a new electric razor, a pet food bowl, toy for grandkids or pet, etc. Some item we don't have on the list but would be of unique interest to the recipient. In accepting a PAL pack, the nominee agrees to provide a response to receiving the gifts; a thank you note, a quote, a photo enjoying an item received, etc. We want to be able to close the loop with those that donated the gifts by letting them know that their generosity was well received and had a positive impact. We don't need to know more than first name, gender and age of the nominee in order to collect gifts to include in their PAL pack.
